What it covers

• A registered nurse assigned to you who stays with you for the rest of your cancer journey.
• Coordination of care and benefits for patients with new and existing cancers.
• Support, advice and resources during and after treatment.
• Help understanding your options and the questions to ask your care team.
• A survivorship program after treatment ends — follow-up, lifestyle guidance, help returning to work, and regular checkups.

The program does not replace or overrule your doctor. The nurses work alongside your care team. Your doctor always makes the final decisions about your care.

How to use it

• Register online at cancercareprogram.com or call (877) 640-9610.
• Once registered, a nurse is assigned to you.
• Register only if you've been diagnosed. If you had cancer previously and are now cancer-free, you can still register as a survivor.

What it costs

Nothing. CancerCARE is a free, fully integrated part of your health plan.
