How to use it

Call the navigators before you schedule anything. This is the step that makes the benefit work — if you book the procedure yourself first, it goes through your regular plan instead.

• The navigators find you a participating provider and arrange the procedure.
• You pay nothing at the point of care.

What it costs

Nothing. The benefit is classified as an out-of-network benefit and the claim is paid at 100%. Claims paid through this program do not apply to your in-network deductible, copays or coinsurance.
