The deductible is waived for preventive services, so cleanings and exams cost you nothing at either network level.

Preventive — you pay nothing
- Oral evaluations — 1 in any 6 months
- Routine cleanings — 1 in any 6 months
- Fluoride — children under 19
- Sealants — permanent molars, children under 14
- Bitewing x-rays — 1 in any 12 months
- Full series x-rays — 1 in any 60 months
Basic — you pay 20%
- New fillings
- Simple and surgical extractions
- Space maintainers (children under 19)
- Endodontics including root canal
- General anaesthesia where medically required
- Periodontal treatment
Major — you pay 50%
- Dentures and bridges
- Inlays, onlays and crowns
- Stainless steel crowns (children under 19)
Orthodontia — you pay 50%
Limited to dependent children, $1,000 lifetime maximum per child.

- Find an in-network dentist at sunlife.com/findadentist. Select plan type PPO Plans and network Sun Life Dental Network, then enter your ZIP code.
- Printed ID cards are not mailed. Download yours from your Sun Life account at sunlife.com/account, or use the Sun Life Dental (U.S.) mobile app.
- Share your dental ID card with your dentist at your next visit.
- Is your dentist out of network? You can nominate them at sunlife.com/findadentist.
- Mid-treatment when coverage started? Call dental claims on (800) 442-7742 before your next visit.
24/7 virtual dental visits
Sun Life PPO dental members have access to virtual dental visits through teledentistry.com/sunlife. A consultation may count as a problem-focused exam under your plan, and services may not be available in all states.
Who's eligible
Full-time employees, their spouse and dependent children to age 26.
Worth knowing: the dental plan is integrated with the Sun Life voluntary Accident and Critical Illness policies. If you have either of those and get your teeth cleaned at a participating provider, a $50 wellness benefit is paid automatically for each — see Supplemental Coverage.
